This study aims to assess the level of Total Quality Management (TQM) practices and perceived organizational performance, and to examine the relationship between them from the perspective of employees at Bani Walid General Hospital. A descriptive-analytical cross-sectional design was adopted, analyzing 60 complete questionnaire responses comprising 18 items (12 items for six TQM dimensions and 6 items for perceived performance). Data were analyzed using frequencies, percentages, means, standard deviations, Cronbach's alpha, Pearson and Spearman correlation coefficients, simple linear regression, and a supplementary permutation test. The mean score for TQM practices was 4.049 (SD = 0.261), while perceived organizational performance averaged 3.500 (SD = 0.718), both falling within the high evaluation level. Results revealed a statistically significant, weak-to-moderate positive correlation between overall TQM and perceived performance (r = 0.282, p = 0.029), with the regression model explaining 7.97% of the variance in performance scores. Among the six sub-dimensions, only "reliance on information" demonstrated a statistically significant correlation with performance. However, Cronbach's alpha for the TQM scale was 0.563, with some subscales exhibiting poor internal consistency. Consequently, these findings should be interpreted as exploratory indicators constrained by measurement quality, rather than causal evidence or an evaluation of clinical performance. The study recommends re-evaluating the measurement instrument, enhancing patient-needs monitoring, linking employee evaluations with documented operational indicators, and implementing measurable incremental improvements.
